In Lesotho, Pact is providing grants to improve community-based care and support services to People Living with HIV/AIDS (PLHIV) and to implement prevention interventions in collaboration with NGOs, CBOs and FBOs with USAID's HIV/AIDS Program. Pact also provides partners with high-quality technical expertise to assist them in delivering effective programming, as well as capacity building services to increase their ability to provide HIV/AIDS services.

Grants are awarded through a competitive Annual Program Statement process, soliciting both nascent and established organizations in Lesotho. Grants go primarily to organizations working through abstinence and being faithful and comprehensive palliative care. This includes support for caretakers and other members of vulnerable households, and Orphan and Vulnerable Children care and support, such as the provision of meals. Pact also supports partner organizations in the distribution of condoms and the implementation of referral services for children to educational support (school fees, scholastic materials), psycho–social counseling, health care referrals and follow-ups for treatment adherence for antiretroviral therapy.
Furthermore, Pact partners train cadres of local adherence coordinators in home-based care, who conduct home-based care visits to people living with HIV. These coordinators provided services during home-visits such as psycho-social counseling and support, managing physical pain, follow-ups for antiretroviral adherence, general hygiene, and referral to health facilities for other medical support.  In addition to the above services, Pact partners provide support to people living with HIV/AIDS through strengthening of HIV support groups. Members of support groups are provided with information of HIV/AIDS, adherence to drugs, and positive living.
